United Health Centers of the San Joaquin Valley: Expanding Healthcare and Opportunity for Older Adults

NFF is proud to finance our first PACE project with United Health Centers of the San Joaquin Valley, deploying $12 million in New Markets Tax Credits (NMTC) to build a new PACE Center in Fresno, CA.

 For many older adults in California’s San Joaquin Valley, staying healthy can be a constant uphill battle. Chronic health conditions, rising medical costs, and a shortage of healthcare providers often delay or limit access to timely, coordinated care. Limited community resources further challenge older adults’ ability to maintain their independence – particularly in Fresno and Kern Counties, where poverty impacts more than half of the older adult population.

To meet this urgent need, NFF provided a $12 million NMTC allocation to United Health Centers of the San Joaquin Valley (UHC) to support the development of a 20,560-square-foot Program of All-Inclusive Care for the Elderly (PACE) Center in Fresno, California. By creating a local hub for comprehensive care, the project directly addresses the complex health and social needs of older adults and expands access to critical services in the region.

“Our health centers offer medical, dental, behavioral health, and supportive services in one place, making it easier for individuals and families – especially those facing barriers – to receive the care they need without navigating multiple systems,” said Jesus Maldonado, UHC Executive Vice President and Chief Financial Officer.

The $13.4 million total project cost will fund the construction and launch of the new PACE Center, bringing integrated medical care, therapy, meals, transportation, and caregiver support to an estimated 350 low-income older adults each year. In addition to improving health outcomes, the project is expected to generate over 100 construction jobs and more than 100 permanent employment opportunities, contributing to local economic growth and workforce stability.

Maldonado continued, “This financing will help us maintain and enhance critical services for our most vulnerable patients while also positioning us to scale care where it’s needed most. It supports our mission to ensure consistent access to high-quality, culturally responsive care for the communities we serve.”

As NFF’s first financed PACE project, this initiative represents a significant milestone in expanding community-centered health investments in central California. More than a healthcare facility, UHC’s new PACE Center will foster connection, dignity, and well-being for older adults while strengthening the overall health and vitality of the surrounding community.
